We are starting to think about re-commissioning Short Breaks and would like to gather your feedback to inform our planning. Short Breaks is a service is for disabled children and young people to provide outcome-based opportunities and experiences through fun activities whilst providing parents and carers with a "break" from their caring responsibilities.

The questions should take about 10-15 minutes to complete. We value your opinions and want to know where you think we can improve the various aspects of the process.
