Welcome to ‘My Voice’ a survey that gives you the chance to share your views, thoughts, and feelings. By answering the questions as honestly as you can it will help the professionals working with you in school to better understand how you are feeling and how to support you with your education. You are very welcome to complete the survey alone or with a trusted adult if you think this will help you.
The questions in this survey help to create a more accurate picture of your life so the more detail you provide the better. The questions cover a range of topics spread over 5 short sections, including school, home, health and hopes for the future but don’t worry there are no right or wrong answers, just your thoughts and feelings. If you are unsure how to answer a question or you just don’t want to answer, then you can leave it blank. Just remember the more questions you answer and detail you provide, the more it will help to better shape the future support that professionals may be able to provide for you.
Gloucestershire County Council will be the Data Controller for all personal data received as part of this survey. Any information you provide will be treated as confidential and in accordance with UK Data Protection Legislation. Our lawful basis for collecting your information is Public Task – The Childrens and Families Act. We will keep and use your data for the purpose of assessing, informing professional conversations and providing appropriate support to you.
