INFECTION CONTROL & TESTING FUND – ROUND 3 (ICTF3) AND OMICRON SUPPORT FUND (OSF) GLOUCESTERSHIRE CARE HOME FINANCIAL RETURN SURVEY

Please note the following:

• The questions in the first section below are about the ICTF3 grant funding for Infection Prevention & Control and Vaccines. The total of all the monthly amounts you submit for these questions should be equal to the total amount that your care home has spent from the 2 grant payments it received from the ICTF3 for both (1) Infection Prevention & Control and (2) Vaccines.
• The questions in the second section below are about the ICTF3 grant funding for Testing. The total of all the monthly amounts you submit for these questions should be equal to the total amount that your care home has spent from the 2 grant payments it received from the ICTF3 for Testing.
• The questions in the third section are about the OSF grant funding. The total amounts you submit should be equal to the total amount that your care home has spent from the single grant payment from the Omicron Support Fund.
