Primary School Nominations
Thank you for agreeing to be part of our Early Help Transition Project this year. Please find below the survey where you are able to nominate the students you feel would benefit from the additional support on offer. At this point, we are asking for nominations of Year 6 pupils who:
receive no intervention from external agencies or sources;
would benefit from friendship and/or relationship support;
have difficulty forming new relationships;
find change or new situations challenging;
have poor attendance due to anxiety and/or poor social skills;
have difficulty with peer relationships, which could include bullying.
We will be accepting children with a My Plan in place where the above needs are identified.The offer does not extend to those children with an EHCP or statutory Children’s Service intervention.
We are keen to reach more young people this year and to fill the places that are available at each secondary school (around 20 young people at each secondary school) and so would like you and your team to begin to think about your Year 6 pupils who would benefit from the project.
Please click on "take survey" to complete the form below to nominate up to three pupils for the project. The survey will need to be completed in full for each pupil. We will then be in touch to offer out places, seek parental consent and gather information about the young people’s needs. If you feel that more than three pupils from your setting would benefit, please contact Julie Jones to discuss this.
The information you give us will be used for the Transition Project only and will be deleted when the project is completed.
