Adult social care infection control fund (round 2) and rapid testing fund Gloucestershire care home provider survey

Please note the following:

• The first set of questions below are about the Infection Control Fund (Round 2). The total of all the monthly amounts you submit for these questions should be equal to the total amount that your care home has spent from 3 grant payments it received from the Infection Control Fund (Round 2).
  
  
• The second set of questions below are about the Rapid Testing Fund. The total of all the monthly amounts you submit for these questions should be equal to the total amount that your care home has spent from 2 grant payments it received from the Rapid Testing Fund.
